This is a Gentex Corporation U.S. Army Advanced Combat Helmet (ACH) Size Large With Cover & Night Vision Goggles Mount MSN: 8470-01-529-6344
It is in pretty good shape was used by someone in the Army not sure where he was stationed or served. The is a couple spots on the cover from where the screws had a little rust that might clean off and I think you can get replacement screws.
